OCD Anxiety Centers – SMS/MMS Terms and Conditions

1. Overview and Consent
By providing your mobile phone number to OCD Anxiety Centers (“we,” “us,” “our”), you consent to receive SMS or MMS messages from us. These may include appointment reminders, important updates, service information, and communications related to your care and our products and services. Message frequency varies and may be up to 100 messages per month. Message and data rates may apply. Consent to receive text messages is not a condition of purchase. 2. HIPAA and Protected Health Information
OCD Anxiety Centers is a HIPAA-covered entity. We will not send protected health information (PHI) by SMS without your explicit written consent. SMS is not a fully secure method of communication and may carry privacy risks. By opting in, you acknowledge and accept the privacy risks associated with receiving PHI via SMS. 3. Providing and Updating Your Number
You confirm that the mobile number you provide is accurate and that you are the authorized user or subscriber. If your number changes, you must notify us immediately at admissions@ocdanxietycenters.com or (385) 309-8403 before the change takes effect. (TCPA; FCC Rules on Reassigned Numbers)
4. Automated and Non-Automated Messages
By providing your number, you consent to receive recurring automated and non-automated text messages from OCD Anxiety Centers. (47 U.S.C. § 227)
5. Opt-Out Instructions
You may opt out at any time by replying with one of the following commands: STOP, END, CANCEL, UNSUBSCRIBE, or QUIT. You will receive one final message confirming your opt-out. You may also opt out by emailing admissions@ocdanxietycenters.com. Opting out will stop all non-emergency SMS communications but may not stop legally required or emergency health alerts. 7. Carrier Disclaimers
Messages are sent from 801-630-0144. Service is available to U.S. residents only. Participating U.S. carriers include AT&T, T-Mobile®, Verizon Wireless, Sprint, Boost, U.S. Cellular®, MetroPCS®, InterOp, Cellcom, C Spire Wireless, Cricket, and Virgin Mobile. Not all carriers may support the service. T-Mobile is not liable for delayed or undelivered messages. (CTIA Carrier Requirements)
8. Data Collection and Use
We may collect your phone number, carrier name, date, time, and message content, as well as other information you provide. This information is used to deliver services you have requested and for operational purposes. For more details, see our Privacy Policy at https://www.ocdanxietycenters.com/privacy-policy/. (CCPA Cal. 9. Retention of SMS Consent
We maintain records of SMS consent for at least four (4) years or as otherwise required by law. (TCPA recordkeeping best practice; 28 U.S.C. § 1658)
10. Service Modifications and Termination
We may change or discontinue our SMS service at any time, with or without notice.
11. Indemnification
You agree to indemnify and hold harmless OCD Anxiety Centers from claims related to providing a number you do not own, failure to update your contact information, or violations of the Telephone Consumer Protection Act (TCPA) or similar state laws.
12. Eligibility
By participating, you confirm you are at least 18 years old, have a device capable of two-way messaging, and are a customer of a participating wireless carrier.
